Post-Surgical Wound Care at Home

Recover from surgery at home — with a nurse practitioner watching your incision so complications get caught early.

Recover at home, with expert eyes on your incision

Recovery is easier at home — but a surgical wound still needs watching, and the first days and weeks are when complications are most likely. Instead of asking you to travel back and forth while you’re healing, we come to you. A nurse practitioner monitors your incision, changes dressings, and catches trouble early.

What we manage

  • Surgical incision care and dressing changes
  • Monitoring for signs of infection
  • Wound dehiscence (a reopened surgical wound)
  • Guidance to support healing and reduce complications
  • Walking alongside your surgeon and home health team

Fresh out of the hospital? We move fast

We don’t like to wait around. For stat referrals we’re often out the next day — and we keep your surgeon and home health nurses in the loop the whole way, until your wound graduates.

What a visit looks like

Each visit, your nurse practitioner checks the incision, changes the dressing, watches for the early signs of infection, and confirms the wound is closing the way it should. We follow the surgeon’s instructions, document each visit so the surgical team can see progress, and tell you plainly how recovery is tracking. If you’re worried about doing something wrong at home, we’ll walk you and your family through the dressing and what’s normal to expect — so nobody’s guessing between visits.

Signs to call us about

Most incisions heal without trouble, but a few changes are worth a call to us or your surgeon right away:

  • Increasing redness, warmth, swelling, or pain around the incision
  • Drainage that increases, turns cloudy or yellow-green, or starts to smell
  • The wound edges pulling apart, or the incision reopening (dehiscence)
  • A fever, chills, or simply feeling unwell

You don’t need to diagnose anything — just tell us what you’re seeing, and we’ll take it from there. That’s exactly why we come to you: so a small problem gets caught early instead of turning into a trip back to the hospital.

We verify your benefits first — see how coverage works, or request a visit and we’ll come to you.

FAQ

Frequently asked questions

Do you manage incisions and dehiscence at home?
Yes. We care for surgical incisions at home — dressing changes, watching for infection, and managing wound dehiscence (a reopened wound) if it happens.
How quickly can you start after a hospital stay?
Fast. Stat referrals are often seen the next day, sometimes the same day. Send us the orders and we'll move — no waiting around.
What do you need from my surgeon?
Just the orders and any wound instructions from the hospital or surgical team. We take it from there, keep your surgeon updated, and reach out if anything about the incision concerns us.
How long will you follow the incision?
Until it graduates — until the wound is closed and healing well on its own. Most surgical wounds are a shorter course than chronic ones, but we stay as long as it takes and we don't disappear before you're ready.
