Recovery is easier at home, but a surgical wound still needs watching, and the weeks after a procedure are when complications are most likely. Instead of asking you to travel back and forth while you’re healing, we come to you. A nurse practitioner monitors your incision, changes dressings, and catches trouble early.

When we step in. We begin post-surgical wound care once your surgeon has formally discharged you from their post-operative care. We don’t manage active, surgeon-directed post-op wounds. When the surgical team has handed off and the wound still needs attention to finish healing, that’s where we come in.

Once your surgeon has discharged you from post-op care, we don’t like to wait around. For stat referrals we’re often out within a day, and we keep your surgeon and home health nurses in the loop the whole way, until your wound is fully healed.

What a visit looks like

Each visit, your nurse practitioner checks the incision, changes the dressing, watches for the early signs of infection, and confirms the wound is closing the way it should. We follow the surgeon’s instructions, document each visit so the surgical team can see progress, and tell you plainly how recovery is tracking. If you’re worried about doing something wrong at home, we’ll walk you and your family through the dressing and what’s normal to expect, so nobody’s guessing between visits.

Signs to call us about

Most incisions heal without trouble, but a few changes are worth a call to us or your surgeon right away:

  • Increasing redness, warmth, swelling, or pain around the incision
  • Drainage that increases, turns cloudy or yellow-green, or starts to smell
  • The wound edges pulling apart, or the incision reopening (dehiscence)
  • A fever, chills, or simply feeling unwell

What do you need from my surgeon?
Your discharge from post-op care, plus any wound orders from the hospital or surgical team. Once your surgeon has released you, we take it from there, keep them updated, and reach out if anything about the wound concerns us.
How long will you follow the incision?
Until it's fully healed, the wound closed and healing well on its own. Most surgical wounds are a shorter course than chronic ones, but we stay as long as it takes and we don't disappear before you're ready.
